Different things can cause physical pain. WebbA patient’s pain must be assessed on an individual basis and recorded systematically, involving the patient where possible. A pain scale of 0-3 should be used to classify pain, where 0 = no pain 1 = mild pain 2 = moderate pain 3 = severe pain. This pain score corresponds to the three steps in the WHO analgesic ladder.
